| 備註 | 註：1.適用範圍：(1)修正式鼻竇手術(Revision sinus surgery)。(2)因先天鼻竇發育異常、術後或外傷造成之鼻竇解剖扭曲(Distorted sinus anatomy of development, postoperative or traumatic origin)。(3)廣泛性鼻竇息肉症(Extensive sinonasal polyposis)。(4)病變涉及額竇、後篩竇或蝶竇(Pathology involving the frontal, posterior ethmoid or sphenoid sinuses)。(5)病變鄰近顱底、眼眶、視神經或頸動脈附近(Disease abutting the skull base, orbit, optic nerve or carotid artery)。(6)腦脊溢漏或顱底缺陷(CSF rhinorrhea or conditions in which there is a skull base defect )。(7)良性、惡性鼻竇或顱底腫瘤(Benign, malignant sinonasal or skull base neoplasms)。2.含鼻外及口內徑路。3.一般材料費及單次使用器械追蹤器或病人追蹤器，得另加計百分之一百十二。4.併同申報83081B或83082B。 |
